Overview

Every form should have a declaration page before submission. If you are building the form on iCM, you can use the ‘Declaration snippet’ otherwise the page must have one checkbox which declares the information is accurate, you agree to any terms and conditions and the council’s privacy policy as demonstrated below.

How it works

I confirm that the information I have given is correct.

I will tell you immediately if my details (for example address) or my circumstances change.

I confirm I have read and understood the Privacy notice.

If you do not wish Somerset Council to hold this information you cannot continue. Therefore, no personal details will be collected and no further action will be taken.

Things to remember

Any navigation buttons such as ‘Submit’ or ‘Next’ must be a conditional display based on the toggle of the checkbox.

Each service area should have its own privacy policy page on https://www.somerset.gov.uk, this must be linked to in the declaration. If this page does not exist it must be created before a form can be put live.
